Colorado DR 2401: Confidential Medical Exam Report

Colorado's Division of Motor Vehicles uses this confidential report to learn about a driver's medical conditions and how much they impair the driving task. Based on the clinician's assessment, the DMV decides whether to license the driver, add restrictions or require re-evaluation. It is a long form with its own instruction sheet, and everything after the Driver/Patient Section is the clinician's.

Who completes it

A physician or advanced practice registered nurse completes all sections after the Driver/Patient Section; the form's immunity clause also names physician assistants. The driver fills in the Driver/Patient Section.

Submitting and payment

How to submit it
Fax or mail the completed report to the Colorado DMV in Denver, using the fax number and post office box printed on the form.
Who pays
The form lists no fee.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can complete Colorado form DR 2401?

A physician or an advanced practice registered nurse completes the medical sections, which are everything after the Driver/Patient Section. The form's good-faith immunity clause under C.R.S. 42-2-112 also refers to physician assistants. The driver completes only the Driver/Patient Section at the start.

How do I send DR 2401 to the Colorado DMV?

Fax it or mail it to the Colorado Division of Motor Vehicles in Denver. The fax number and post office box are printed on the form itself. If vision also needs assessment, the eye examiner sends the separate DR 2402 Confidential Eye Examination Report the same way.

Is a clinician protected when reporting a driver on DR 2401?

The form cites C.R.S. 42-2-112, which protects clinicians who give a medical opinion in good faith, and its immunity clause names physicians, advanced practice registered nurses and physician assistants. The licensing decision rests with the DMV, which uses the report to license, restrict or re-evaluate the driver.
