Colorado DR 2402: Confidential Eye Exam Report

Colorado's DMV uses this eye report to decide whether a driver's vision supports licensing, with or without restrictions. The examiner records progressive eye conditions, visual field deficits, distance acuity with and without correction and through bioptic lenses, and horizontal fields, then picks one fitness-to-drive finding and any license restrictions.

Who completes it

The driver completes the Driver/Patient Section. The examiner section must be completed and signed by an MD or DO, PA, APRN, or an ophthalmologist or optometrist, who must also give specialty, license number and phone.

Submitting and payment

How to submit it
Fax or mail it to the Colorado DMV in Denver at the fax number and post office box printed on the form. The form is valid for 180 days from the exam date.
Who pays
The form lists no fee.

Good to know

  • Select exactly one fitness option: fit, not fit, fit contingent on a DMV road test, or determination pending with a rehab permit; alterations, erasures or multiple selections invalidate the form.
  • If the DMV requested the form, the exam must take place after the issue date of the notice of cancellation and denial.
  • Colorado recommends 20/40 or better in either eye and 120 degrees of combined horizontal field, or 60 degrees in the remaining eye if blind in one.
  • Leaving 'Require DMV retesting in one year?' blank defaults to No; restrictions include daylight only, mile radius, no highway, bioptic lens, automatic transmission and restricted speed.
