Nevada DMV DP-18 Eye Examination Certificate

Applicants who fail Nevada DMV's vision screening take this certificate to a physician or optometrist for an eye exam. It records acuity without correction, with the current prescription and with any new one, whether glasses are being fitted and any progressive abnormality. The applicant must present it to DMV within 90 days of the date the examination was performed.

Who completes it

A physician or optometrist licensed to practice in Nevada records the exam results, answers the impairment questions and signs; the applicant fills in identity details and also signs the certificate.

Submitting and payment

How to submit it
The applicant presents the completed certificate to DMV within 90 days of the examination; the form is issued by DMV's Driver's License Assessment Team.

Good to know

  • The certificate cites NAC 483.310 and 483.340, the Nevada regulations it is issued under.
  • A third column records acuity with a new prescription if one is being issued, alongside yes/no answers on whether glasses could correct the deficiency and whether glasses are being fitted.
  • A 'yes' to progressive abnormalities, or to impairment of safe driving, calls for a written explanation of the case and recommended restrictions.
  • Applicants who want to drive with bioptic telescopes use DLD-98 instead, which carries its own required restriction list.

Frequently asked questions

How long is Nevada's DP-18 valid?

The applicant must present it within 90 days of the date the examination was performed. The exam must be done by a physician or optometrist licensed to practice in Nevada, as the certification line and the note at the bottom of the form both state.

Can an optometrist complete Nevada's Eye Examination Certificate?

Yes. The certification line is for a physician or optometrist licensed to practice in Nevada, who prints their name, certifies having examined the applicant, and signs with office address, phone number and the date of examination.

What does DP-18 ask beyond acuity?

Whether the deficiency could be corrected with glasses, whether glasses are being fitted, whether there are progressive abnormalities, and whether the condition will impair the applicant's ability to drive safely. 'Yes' answers to the last two call for an explanation and recommended restrictions.
