Nevada DMV DLD-7 Confidential Physicians Report

Nevada DMV relies on this two-page report to judge whether a medical condition affects a motorist's ability to drive safely. The clinician gives the diagnosis and its status, medications, adherence, any seizure or loss-of-consciousness history and recommended restrictions. Under the Nevada Administrative Code, DMV must receive the report within 30 days after the date of the examination.

Who completes it

A physician, APRN or physician assistant completes items 1 to 12 and signs with a license number; the patient signs the page 2 release authorizing clinicians, clinics and hospitals, including the VA, to share related medical information.

Submitting and payment

How to submit it
Send it to Nevada DMV so it arrives within 30 days after the examination date; the DMV contact block printed on the form lists the Carson City office and a fax line.

Good to know

  • Every field is mandatory. A 'yes' or 'uncertain' on whether the condition affects safe driving, and a 'yes' on medication effects, each need a written explanation.
  • Item 10 asks about loss or lapse of consciousness, seizures, fainting or dizzy spells, the date of the last episode, whether it was isolated and whether more seizures are likely.
  • Page 2 lets the patient request a medical condition indicator on the license or ID, such as epilepsy, diabetes, autistic disorder or anticoagulant use, which the physician must state on the form.
  • To get the indicator, the patient presents the form in person at DMV. Adding it is free, though the usual card production fee for a new card applies.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How long does a Nevada clinician have to send the DLD-7?

The form states that, under the Nevada Administrative Code, DMV must receive the Confidential Physicians Report within 30 days after the date of the examination. The date of examination goes next to the signature, and item 4 also asks how long the person has been your patient and the date of the last examination.

Can an APRN or PA sign Nevada's Confidential Physicians Report?

Yes. The signature line is for an authorized physician, APRN or PA, with the printed name, license number, office phone and office address of whoever signs. The patient release on page 2 likewise names physicians, surgeons, advanced practice registered nurses and physician assistants as sources of medical information.

What medical indicators can go on a Nevada license through the DLD-7?

The optional section lists anticoagulants, autistic disorder, blindness and low vision, chronic airway obstruction, coronary atherosclerosis, deafness, difficulty in walking, diminished hearing, epilepsy, food allergies, malignant hyperthermia and diabetes, each with a code. Only one can be checked, and the physician must state that the patient has it.
