Nevada DO 200 Window Tint Medical Exemption

Nevada drivers who need darker window tint than the law allows apply to the Highway Patrol with this three-sheet application. For a medical exemption, a Nevada-licensed physician gives the clinical diagnosis, a recommended visible light transmittance (VLT) percentage and whether the permit should last four years or indefinitely. If approved, the original comes back to the applicant and must stay in the vehicle.

Who completes it

The applicant completes Section 1 (personal details, up to two vehicles, reason) and signs; a currently licensed Nevada physician completes and signs Section 2. Section 3 is only for ambulance, hearse and law enforcement exemptions.

Submitting and payment

How to submit it
The applicant submits the completed application to the Nevada Highway Patrol, Department of Public Safety; the form appears on Nevada DMV's forms page, but NHP decides it.

Good to know

  • The form states that no window tint exemption below 20% VLT will be approved by the Department of Public Safety, whatever percentage is recommended.
  • Choose a 4-year permit for a temporary condition or indefinite for a permanent, stable one. The approval, signed by the DPS Director and NHP Chief, states the duration and approved VLT.
  • The exemption is not transferable. If a listed vehicle is sold, the approval letter must be returned to the Department of Public Safety.
  • The application cites NRS 484D.440 and NAC 484D.280 to 484D.290. The applicant certifies being the primary driver of every listed vehicle, and false statements are a felony.

Frequently asked questions

Who can certify a medical window tint exemption in Nevada?

Section 2 must be completed by a currently licensed State of Nevada physician, who describes the exact nature of the impairment and signs a declaration that tinting the listed vehicles is a medical necessity for the applicant's condition, adding a license number and contact details.

What VLT can a Nevada physician recommend on DO 200?

Section 2 asks for a recommended percentage of visible light transmittance for the applicant. The form states that the Department of Public Safety will not approve any window tint exemption below 20% VLT, and the approved percentage is written on the final page.

How long does a Nevada medical tint exemption last?

The physician recommends either a 4-year permit for a temporary condition or an indefinite permit for a permanent, stable one. DPS approves it for 4 years or indefinitely, and the approved letter, or a copy, must be carried in the vehicle at all times.
