VIAtrans Application: Medical Section (San Antonio)

VIA Metropolitan Transit's VIAtrans application decides who can use its curb-to-curb ADA paratransit service in San Antonio. Pages 5 to 7 are the medical section, completed by a doctor or other medical professional familiar with the disability: the main barrier to riding the bus, assistive devices, safety for assessment tasks, functional limitations and ICD-10 codes.

Who completes it

The applicant completes pages 1 to 4 and must leave pages 5 to 7 blank. A doctor or specialist providing treatment completes the medical section, initials any corrections, enters NA or DK instead of leaving blanks, and signs with license number and specialty.

Submitting and payment

How to submit it
The original application is mailed or delivered to VIA's Accessible Services Department; VIA does not accept copies, faxes or email.

Good to know

  • If pages 5 to 7 already have entries when the form arrives, the clinician should not complete it; VIA does not process applications where the applicant wrote in the medical section.
  • Part B asks whether a 12-inch step with handrails, walking 2,640 feet on varied terrain, or independent wheelchair travel is safe for the applicant, with reasons for any unsafe answer.
  • Part C rates walking, standing, stepping, communicating, remembering, planning and interacting with others from no limitation to severe, with explanations.
  • VIA may request more information or a functional assessment, and approves unconditional, conditional or short-term (3 to 12 months) service, with an appeals process.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Is the VIAtrans application available in Spanish?

Yes. VIA publishes the VIAtrans Service Application in English and Spanish (Aplicación de Servicio de VIAtrans), both dated January 2023. The medical section has the same structure in each, and the applicant can mark Spanish as the language of choice on page 1.

What should a clinician do if the patient already wrote on the VIAtrans medical pages?

The instructions tell medical professionals not to complete any part of the application if pages 5, 6 or 7 have entries when delivered to the office. VIA will not process an application where the applicant completed any part of the medical section, and heavily edited applications are also rejected.

Can VIAtrans paperwork be faxed or emailed?

No. VIA requires the original application and prints 'do not submit copies, do not fax, do not email' on the form. The completed original goes to the Accessible Services Department at the VIA Metro Center by mail or delivery. Applicants should hear within 21 days of a complete application.
