DART Physician Verification Form: Visual Impairments

DART uses this one-page version of its physician verification for paratransit applicants whose disability is visual. The eye doctor names the eye disease or condition and its onset date, records visual fields or best-corrected acuity for each eye and both eyes, and gives a prognosis. The form is designed to confirm that the patient is legally blind in both eyes.

Who completes it

A licensed ophthalmologist (MD) or optometrist (OD) who currently treats or formerly treated the patient completes and signs it with license number and state. The applicant completes DART's certification form for a visual disability.

Submitting and payment

How to submit it
The form is returned to DART Paratransit Services with the visual-disability certification form by mail, fax or email.

Good to know

  • The doctor marks the impairment as permanent or states the number of months it is expected to last.
  • Every item must be filled in; DART does not process incomplete forms and returns them to the patient.
  • Applicants with a visual disability go through a visual assessment as part of DART's in-person eligibility assessment.
  • Applicants with conditions other than vision loss use DART's general Physician Verification of Disability Form instead.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
