CapMetro MetroAccess Part B: Provider Verification

Austin's Capital Metro requires Part B of its ADA Paratransit Service Application before an applicant has an in-person MetroAccess eligibility review. The treating provider gives written diagnoses with ICD-10-CM or DSM codes, seizure history, visual acuity and fields, mobility devices and a blood pressure clearance for the functional assessment. CapMetro decides within 21 days of the complete application and review.

Who completes it

The applicant completes Part A and may not complete Part B. A licensed or registered provider completes all of Part B: examples include physicians, PAs, nurses, NPs, podiatrists, chiropractors, optometrists, licensed therapists, social workers, psychologists, counselors, certified O&M specialists and special education teachers.

Submitting and payment

How to submit it
Paperwork can be mailed, faxed or emailed to CapMetro's Eligibility Department, but the original Part B must be brought to the in-person review if it was not mailed; CapMetro can also send the provider an online Part B link.

Good to know

  • Part B requires an original hand signature, not a signature stamp, and any form with requested information altered or omitted is returned to the applicant.
  • A resting blood pressure of 180/110 or higher suspends the functional assessment unless the treating provider has entered a ceiling under which the applicant may proceed.
  • The functional assessment, run by a licensed therapist, can involve walking up to 3/4 mile, which is why the hypertension question matters.
  • Disability or age alone, or distance to and from a bus stop, does not by itself qualify a person for MetroAccess.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Can a patient fill out MetroAccess Part B?

No. The instructions state that the applicant may not complete Part B, and the treating professional must fill it out in its entirety. Any form with requested information altered or omitted is not processed and is returned to the applicant, which delays the eligibility review.

Will CapMetro accept a faxed or scanned Part B?

Part B is printed with 'Copies, faxes or scans will not be accepted' and needs an original hand signature. CapMetro does let applicants send paperwork by fax or email to start the process, but the original must then be brought to the in-person eligibility review.

Why does MetroAccess Part B ask about blood pressure?

CapMetro takes a blood pressure reading before the functional assessment. If resting pressure is 180/110 or higher, the assessment stops until a provider clears the applicant. A provider already treating hypertension can enter the reading the applicant may go up to, so the assessment can continue without a referral back.
