DART Paratransit Physician Verification of Disability

Dallas Area Rapid Transit requires written disability verification from a physician as the second step of applying for DART Paratransit Services. This one-page form confirms each diagnosis or disability with its date of onset and records which mobility devices are prescribed. It goes back to DART with the certification form, after which DART schedules an in-person assessment.

Who completes it

The applicant completes DART's separate certification form. A physician or other credentialed provider (MD, DO, RN, PA or FNP) completes this form in its entirety and signs it with license number, state and office phone.

Submitting and payment

How to submit it
The form is returned with the certification form to DART Paratransit Services by mail, fax or email; a DART representative then schedules the in-person assessment.

Good to know

  • Any omitted item means the form is not processed and is returned to the patient, so every line, including onset dates, should be filled.
  • The device section asks whether a manual or powered wheelchair, powered scooter, walker with or without a seat, or cane is prescribed.
  • DART notifies applicants within 21 days of the eligibility assessment; if no decision is made within 21 days of receiving the application, presumptive eligibility applies.
  • DART has separate verification forms for visual impairments, seizure and epilepsy, and behavioral health or intellectual disabilities.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the DART Physician Verification of Disability Form?

The signature line asks for the physician's name and credentials as MD, DO, RN, PA or FNP, with license number and state. The form's purpose is to verify the medical conditions that prevent the applicant from using DART's bus and rail services, so it should come from a treating clinician.

What happens after the DART physician form is submitted?

A DART representative schedules an in-person assessment with an Eligibility and Training Specialist, at no charge. It includes a brief interview, a photo, and a functional, cognitive or visual assessment depending on the disability. DART then grants conditional or unconditional eligibility, or denies it with a right to appeal.

Does DART need anything besides this form?

Yes. The applicant must also complete a DART certification form and attend the in-person assessment. DART's page warns that failing to complete any step prevents it from considering the application. Applicants with vision, seizure or cognitive conditions may use DART's condition-specific forms.
