DART Seizure and Epilepsy Verification Form

DART uses this two-page physician questionnaire to judge paratransit eligibility for applicants with a seizure disorder or epilepsy. The physician answers ten questions covering diagnosis and onset, the date of the last seizure, seizure types, medication control, warning signs, triggers, need for emergency care and confusion afterward. DART's eligibility page lists it as the Epilepsy and Seizure Disorder Inquiry.

Who completes it

The applicant enters name, date of birth and the last four digits of the Social Security number. The physician answers every question and signs with printed name and license number and state.

Submitting and payment

How to submit it
The form is returned to DART Paratransit Services by mail, fax or email along with the rest of the paratransit application.

Good to know

  • Every question must be answered; DART returns incomplete forms rather than processing them.
  • Seizure types to circle include absence, petit mal, grand mal, complex partial, simple partial, psychomotor and tonic clonic, with space for other types.
  • Written explanations are needed when seizures are not controlled by medication, when there are warning signs, when emergency care was needed, or when confusion follows a seizure.
  • DART notifies applicants within 21 days of the eligibility assessment, and anyone denied or given conditions can appeal.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who completes DART's seizure and epilepsy form?

The applicant's physician. The form says it is to be completed and signed by the physician and that every question must be answered. The physician prints their name, signs, and gives a license number and state. The applicant only fills in identifying information at the top.

Where does the seizure form fit in DART's application?

DART's eligibility page lists the Epilepsy and Seizure Disorder Inquiry among its application forms, alongside the standard and visual-disability certification forms and the Re-Certification Form. All forms go back to DART Paratransit Services, which then schedules the in-person assessment.
