DART Behavioral Health or Intellectual Disability Form

DART uses this one-page verification for paratransit applicants with a behavioral health condition, an intellectual disability, a pervasive developmental disorder such as autism, or a related condition such as traumatic brain injury. It records the disability type and the date the determination was made or confirmed. DART's eligibility page lists it as the Cognitive or Intellectual Verification of Disability Form.

Who completes it

Either the local intellectual and developmental disability authority, named on the form as the Mental Retardation Authority, with its service coordinator, or a clinician or therapist giving credentials, license number and state, completes and signs it.

Submitting and payment

How to submit it
The form is returned to DART Paratransit Services with the certification form by mail, fax or email.

Good to know

  • A disability type must be checked or listed: schizophrenia, bipolar disorder, depression or another behavioral health condition; intellectual disability; autism-type disorders; or related conditions with the specific type.
  • The printed form still refers to the Texas Department of Aging and Disability Services and a Mental Retardation Authority, older names that staff may need to map to the current local authority.
  • Incomplete forms are returned. Applicants with this kind of disability have a cognitive assessment as part of DART's in-person eligibility assessment.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign DART's behavioral health or intellectual disability verification?

The form offers two signers: the local authority for intellectual and developmental disabilities, identified with its service coordinator and phone, or a clinician or therapist who lists credentials, license number and state. Either one verifies the disability type and the date it was determined or confirmed.

Is this the same as DART's Cognitive or Intellectual Verification of Disability Form?

Yes. DART's eligibility page links to it as the Cognitive or Intellectual Verification of Disability Form, while the printed title reads Verification of Behavioral Health or Intellectual Disability. It is one of the three physician verification forms DART lists, alongside the general and visual-impairment versions.
