NC MVR-37AA: Disability Placard Renewal Application

North Carolina placard holders use MVR-37AA to renew a disability parking placard or a total and permanent disability parking placard. Clinicians meet it when a patient's five-year placard is due for renewal and recertification is required. The provider rechecks the G.S. 20-37.5 criteria that still apply and signs, and the patient renews by mail or at a License Plate Agency.

Who completes it

The applicant completes and signs only the applicant section, listing current placards. When recertification is required, a licensed medical provider completes the medical provider's section, or the applicant presents VA documentation instead.

Submitting and payment

How to submit it
The renewal and fee are mailed to NCDMV in Raleigh by check or money order (allow three to four weeks), or handled same day at a local License Plate Agency.
Who pays
The applicant pays a per-placard renewal fee (limit two); the form does not address payment for the provider's recertification.

Good to know

  • The medical section has two boxes, recertification required or not required, and the provider checks every G.S. 20-37.5 criterion that still applies.
  • Medical recertification is not required for people 80 or older at the time of renewal.
  • Total and permanent placards renew without medical recertification, so the provider's section matters mainly for holders of the standard five-year placard.
  • The placard registration card must stay with each placard. Organizations certified by the State Vocational Rehabilitation Agency that regularly transport people with disabilities may also apply.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

When does an NC placard renewal need a doctor's recertification?

The standard Disability Parking Placard requires medical provider recertification at renewal. It is not required for people 80 or older at the time of renewal, and the Total and Permanent Disability Parking Placard renews without recertification.

Who can recertify on North Carolina form MVR-37AA?

A licensed medical provider completes and signs the medical provider's section with a printed name, license number, address, phone and date. Documentation from the U.S. Department of Veterans Affairs may be presented instead of the provider's recertification.

Can an NC disability placard be renewed the same day?

Yes. Renewals done at a local License Plate Agency are same-day. Mailed renewals go to NCDMV with a check or money order, and the DMV asks applicants to allow three to four weeks to receive the placard.
