NC MVR-37A: Disability Parking Placard Certification

North Carolina residents use MVR-37A to apply for or renew a disability parking placard. A licensed medical provider certifies a mobility impairment defined in G.S. 20-37.5 and picks one of three placards: a five-year placard that needs recertification at renewal, a temporary placard of one to six months, or a five-year total and permanent placard that does not.

Who completes it

The applicant selects one or two placards and completes and signs only the applicant section. A licensed medical provider completes and signs the medical provider's section on the front page, or the applicant presents VA documentation instead.

Submitting and payment

How to submit it
The applicant takes the form and fee to a local License Plate Agency for same-day service, or mails it to NCDMV in Raleigh and allows three to four weeks.
Who pays
The applicant pays a per-placard fee (limit two); the form does not address payment for the medical certification.

Good to know

  • Choosing 'Total and Permanent' spares the patient future recertification; the standard Disability Parking Placard requires the provider to recertify at each five-year renewal.
  • A licensed certified nurse midwife may certify an initial application for a temporary placard only.
  • Temporary placards run one to six months, with the provider choosing the month count. A temporary placard needed past expiration requires a new application and recertification.
  • No recertification is needed for people 80 or older, or deemed permanently disabled, if certification is on file. Placards can be renewed up to 90 days before expiring.

The Disability Parking Placard and the Total and Permanent Disability Parking Placard are each issued for five years. Temporary placards last one to six months, as chosen by the provider, and then expire and should be destroyed.

What is the difference between MVR-37A and MVR-37AA?

MVR-37A is the medical certification for new applications and renewals, where the provider chooses the placard type. MVR-37AA is the renewal form mailed to existing holders, where the provider marks whether recertification is required. MVR-37 is the separate disability license plate application.
