NC MVR-37: Application for Disability License Plate

North Carolina residents use MVR-37 to register a vehicle with disability license plates. The medical provider's section certifies that the applicant meets one of the definitions of 'handicapped' in G.S. 20-37.5, such as being unable to walk 200 feet without resting. The NCDMV issues the plate by mail or through a License Plate Agency, and five-year guardian renewals skip the medical section.

Who completes it

The applicant (the person with the disability) completes and signs only the applicant section, or a guardian or parent signs in their section. A licensed medical provider completes and signs the medical provider's section, adding a license number, or the applicant presents VA documentation instead.

Submitting and payment

How to submit it
The application and fee are taken to a local License Plate Agency or mailed to NCDMV in Raleigh with a check or money order; mailed plates take three to four weeks.
Who pays
The applicant pays the regular plate fee; the form does not address payment for the medical certification.

Good to know

  • Qualifying conditions include walking under 200 feet without rest, needing an assistive device or person, FEV1 or PaO2 thresholds, portable oxygen, AHA Class III/IV cardiac disease and severe arthritic, neurological or orthopedic limits.
  • Blindness qualifies only when certified by a licensed ophthalmologist, optometrist or the Division of Services for the Blind.
  • When a guardian or parent makes the certification, recertification is due every five years, and the medical provider's section is not required for that five-year renewal.
  • The current version is Rev. 04/26. MVR-37A is the companion form for disability parking placards.

Frequently asked questions

Who can certify North Carolina form MVR-37?

A licensed physician, ophthalmologist, optometrist, physician assistant or nurse practitioner, or the Division of Services for the Blind. A disability determination by the U.S. Department of Veterans Affairs can also satisfy the requirement. The provider signs, prints their name and gives a license number and contact details.

How long does it take to get an NC disability plate?

When MVR-37 is mailed to NCDMV with a check or money order, the DMV asks applicants to allow three to four weeks to receive the plate. The application can also be taken to a local License Plate Agency.

Does the MVR-37 need a doctor at renewal?

The medical provider's section is marked as not required for the five-year renewal application. Five-year recertification applies when a guardian or parent made the certification, and it is completed in the guardian or parent section.
