Iowa Rule 12.36 Form 8: Psychiatric Evaluation

After an Iowa court orders a respondent's psychiatric evaluation, the chief medical officer of the hospital or facility reports the results on this form under Iowa Code 229.14. It covers treatment during the evaluation period, medication at the time of evaluation, psychiatric, medical and family history, transmissible disease, diagnosis and dangerousness findings, and recommends one of four outcomes.

Who completes it

The chief medical officer of the hospital or facility completes and signs the report, recording the date and time of the evaluation. The respondent does not complete any part.

Submitting and payment

How to submit it
File it with the Iowa district court in the respondent's case after the evaluation. Electronic filing is mandatory in Iowa courts unless otherwise required or authorized.
