Iowa Rule 12.36 Form 10: Outpatient Periodic Report

When an Iowa court has ordered a respondent found seriously mentally impaired into treatment on an outpatient or other appropriate basis, the treating facility reports back to the district court on this form under Iowa Code 229.15(2). It states whether the condition improved, stayed the same or deteriorated, whether the respondent is complying, and recommends discharge, inpatient care, continued outpatient treatment or another placement.

Who completes it

The facility's medical director, or the psychiatrist or psychiatric ARNP treating the respondent, completes and signs it. An ARNP without psychiatric certification who meets the mental health professional definition in Iowa Code 228.1 may also complete it.

Submitting and payment

How to submit it
File it with the Iowa district court in the respondent's case. Electronic filing is mandatory in Iowa courts unless otherwise required or authorized.

Good to know

  • Item 10 records treatment status: tentatively discharged, in treatment as ordered, or failing or refusing treatment without good cause in your opinion.
  • If you check 10(A) or 11(A), stop and sign; otherwise give the recommended treatment and explain why it is the least restrictive effective option.
  • Recommendations track Iowa Code 229.14(1)(a) to (d), from no further treatment to full-time custody where inpatient treatment is unlikely to help.
  • Sign with a digitized signature or by printing and signing by hand. April 2024 edition, five fillable pages.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the Iowa outpatient periodic report?

The facility's medical director, or the psychiatrist or psychiatric advanced registered nurse practitioner treating the respondent, under Iowa Code 229.15(3)(a). Under 229.15(3)(b), an ARNP without psychiatric certification may sign if they meet the mental health professional definition in Iowa Code 228.1.

What if the respondent is not following the outpatient order?

Check item 10(C) to report that the respondent is failing or refusing to submit to treatment as the court ordered and, in your opinion, has not shown good cause. Then give your recommended treatment and placement and the facts supporting it.

What findings does Iowa Form 10 ask for?

Your opinion on whether the respondent is mentally ill, with diagnosis and symptoms; can make responsible treatment decisions; is likely to injure self or others or inflict serious emotional injury; cannot meet basic needs; and has a history of noncompliance leading to emergency hospitalization or injury.
