Iowa Rule 12.36 Form 4: Report of Examination

After an application for involuntary hospitalization in Iowa, the court designates a physician or mental health professional to examine the respondent under Iowa Code 229.10. This report gives the examiner's judgment on mental illness, treatability, decision-making capacity, dangerousness and inability to meet basic needs, plus whether the respondent can be evaluated as an outpatient or released to a relative during evaluation.

Who completes it

The physician or mental health professional designated by the court completes and signs the whole report. If a qualified mental health professional assisted with the exam, name them and attach their written report.

Submitting and payment

How to submit it
File it with the district court in the respondent's case before the hospitalization hearing; Iowa requires electronic filing unless otherwise required or authorized.
Who pays
Examiners claim their fees separately on Rule 12.36 Form 14b, Claim for Physician Fees.
