Ohio Probate Form 20.2: Certificate of Examination

Ohio probate courts use Standard Probate Form 20.2 in civil commitment cases started by an Affidavit of Mental Illness (Form 20.1) under R.C. 5122.11. A clinician meets it when asked to examine the person named in the affidavit. The certificate states whether the person meets one of the first four affidavit criteria for court-ordered hospitalization or the fifth criterion for community-based treatment, with supporting findings and treatment recommendations.

Who completes it

A psychiatrist signs alone, or a licensed clinical psychologist and a licensed Ohio physician sign together; the physician's signature is required when the psychologist line is signed. Both personally observe and examine the person and review the affidavit.

Submitting and payment

How to submit it
The certificate is filed with the Affidavit of Mental Illness (Form 20.1) in the county probate court.

Good to know

  • The examiner should make sure the box checked on page 1 of the affidavit matches the criterion certified: one of the first four for hospitalization, or the fifth for community-based treatment.
  • Findings should flag conditions needing immediate attention, such as withdrawal symptoms, need for insulin, recent severe head injury or tuberculosis, plus any specific treatment recommendations.
  • The form asks for the patient's Social Security number, date and place of birth, race, sex and marital status; the current edition took effect July 1, 2026.
