Alabama AV-E1: Medical Emergency Absentee Ballot

Alabama voters who need treatment from a licensed physician within 5 days before an election can apply for an emergency absentee ballot on Form AV-E1. The attending physician completes the 'Physician's Report for Medical Emergency' box, describing and certifying the circumstances as an emergency, and the voter or a designee gets the application to the county Absentee Election Manager.

Who completes it

The voter completes the voter information, election and reason sections and signs. The attending physician completes and signs the Physician's Report for Medical Emergency box on page 1, and a designee, if one is used, signs the designee section.

Submitting and payment

How to submit it
The voter or a designee hands the application to the county Absentee Election Manager, or sends it by U.S. mail or commercial carrier, which is allowed only for medical emergencies; the application and voted ballot are due by noon on election day.

Good to know

  • The physician may attach a report on a separate document instead of using the box on the form.
  • A copy of the voter's valid photo ID must accompany the application, and only one application may go in each envelope.
  • The other emergency reasons on the form (employer requirement, caregiving, a death in the family) need hand delivery by the day before the election; only medical emergencies allow mail or a designee.
  • The current version is dated Revised 06.13.2022 and cites Code of Alabama sections 17-11-3 and 17-11-4.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What does the physician write on Alabama's emergency absentee ballot application?

The Physician's Report for Medical Emergency asks the attending physician to describe and certify the circumstances that constitute the emergency, then sign and date it. If the physician's report is on a separate document, the voter attaches it to the application instead.

When is Alabama's medical emergency absentee application due?

The medical emergency process covers treatment by a licensed physician within 5 days of an election. During that period, the Secretary of State says the application and the voted absentee ballot must be returned no later than noon on the day of the election.

Can someone else deliver the application for the patient?

Yes. In a medical emergency the voter can name a designee in the section at the bottom of the form. The designee can hand in the application, receive the ballot and return the voted ballot to the Absentee Election Manager. Mail or commercial carrier is also allowed, but only for medical emergencies.
