SAAQ ENT (ORL) Examination Report (Form 18)

The SAAQ sends this two-page report to a licence holder whose hearing or another ear, nose and throat condition must be assessed for driving. The health care professional records the diagnosis and whether it is changing, tests hearing with a forced whisper and a recent audiogram, describes any vertigo, and notes other ENT factors likely to affect driving.

Who completes it

The examinee signs the authorization; the examining health care professional completes sections 1 to 5 and gives their profession, licence number, how long the patient has been in their care and visits per year.

Submitting and payment

How to submit it
Submit it through the person's SAAQclic account or by mail to the SAAQ's Direction de l'accompagnement et de l'expertise santé.
Who pays
Any fee for completing the form is paid by the examinee and is not reimbursed by the SAAQ.

Good to know

  • If hearing loss is clinically detectable, record whether a forced whisper is heard at 1.5 metres, with and without a hearing aid.
  • Give the average tonal threshold of the better ear at 500, 1,000 and 2,000 Hz from a recent audiogram, with the date of the test.
  • If the person wears a hearing aid, the audiogram must be done with the device the person wears when driving.
  • For vertigo, give the date of the last episode, whether onset is sudden or gradual, and its nature, duration, frequency, severity and treatment.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

What hearing test does SAAQ form 18 require?

Two pieces of evidence: whether the person perceives a forced whispered voice at 1.5 metres, with and without a hearing aid, and a recent audiogram giving the average tonal threshold for the better ear at 500, 1,000 and 2,000 hertz. Hearing aid users are tested wearing the device they drive with.

Does the SAAQ ENT report cover vertigo?

Yes. Section 3 asks whether the person has or has had vertigo, the date of the last episode, whether it comes on suddenly or gradually, and its nature, duration, frequency, severity and treatment. Section 4 covers any other ENT factor likely to affect driving and the functional impairment it causes.

Who pays for SAAQ form 18?

The examinee. The form states that any fees charged for completing it must be paid by the person examined and are not reimbursed by the SAAQ. The completed form goes back through the person's SAAQclic account or by mail to the SAAQ's health assessment directorate.
