SAAQ Road Test Report by an OT (Form 58)

The SAAQ requests this two-page report when it needs road test results to reassess a person's fitness to drive. The occupational therapist records the test date, duration, vehicle and adaptations, prior driver training and the route, then gives a pass or fail result, any rehabilitation potential and conditions to attach to the licence. Unlike form 57, it has no office-based physical or cognitive assessment.

Who completes it

An occupational therapist completes all three sections and signs with their licence number; the person tested signs an authorization for the SAAQ to discuss their medical information with the therapist.

Submitting and payment

How to submit it
Mail it to the SAAQ's Service de l'évaluation médicale et du suivi du comportement; relevant documents and a separate sheet for the route description may be enclosed.
Who pays
Any fee for completing the form is paid by the person undergoing the examination and is not reimbursed by the SAAQ.

Good to know

  • The road test must include situations of increasing complexity, at least various speed limits, familiar and unfamiliar routes, unprotected left turns, and rural and urban roads.
  • If the person passes, the therapist can attach licence conditions such as automatic transmission, power brakes or steering, hand-operated controls, an accelerator on the left or other adapted controls.
  • After a failure, the report states whether the person shows development or rehabilitation potential and whether accompanied driving with a driving school monitor or OT is required.
  • The SAAQ's occupational therapist page says manoeuvres not assessed need a valid reason, since an incomplete assessment will not let the SAAQ rule.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

What is the difference between SAAQ forms 57 and 58?

Form 57 is the six-page functional assessment report covering medical history, vision, limbs, cognition and behaviour plus a road test. Form 58 is a two-page report of the road test alone, which the SAAQ requests when it needs road test results to reassess fitness to drive.

What must the occupational therapist's road test include?

The SAAQ requires a variety of driving situations of increasing complexity, including at least various speed limits, familiar and unfamiliar routes, unprotected left turns, and rural and urban roads. The therapist specifies the route, such as urban, highway or boulevard, and describes how the test was carried out.

Who pays for the SAAQ road test report?

The person being assessed. The form states that any fees charged for completing it must be paid by the person undergoing the examination and do not qualify for reimbursement by the SAAQ. The therapist mails the completed form to the SAAQ's medical evaluation service.
