SAAQ Declaration of Unfitness to Drive (Form 5626)

Section 603 of Quebec's Highway Safety Code lets a health professional report to the SAAQ the name, address and state of health of a person aged 14 or older whom they consider unfit to drive. This one-page form is that report: it identifies the patient and the professional and describes the illness, impairment, limitations and severity behind the opinion. The SAAQ then decides whether to ask for more information or suspend the licence.

Who completes it

The reporting professional completes the whole form and checks their profession: physician, optometrist, psychologist, occupational therapist or nurse; there is no patient signature.

Submitting and payment

How to submit it
Mail it to the SAAQ's Service de l'évaluation médicale et du suivi du comportement, as printed at the bottom of the form.

Good to know

  • Section 605 of the Highway Safety Code, quoted on the form, bars any action in damages against a health professional for having used section 603.
  • The form asks, yes or no, whether the professional has told the person about the report to the SAAQ.
  • The SAAQ does not always suspend on receipt: it often asks for more details and, outside clear-danger cases, must give 20 days' notice before a suspension.
  • The English version (5626A, 2024-03) is marked as intended for health professionals with no address in Québec; the French original is form 5626.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can declare a patient unfit to drive to the SAAQ?

Under section 603 of the Highway Safety Code, a health professional may, within their field of practice, report a person aged 14 or older whom they consider unfit to drive. The form lists physicians, optometrists, psychologists, occupational therapists and nurses, and the law authorizes them to disclose information learned in their practice.

Does the SAAQ suspend a licence as soon as it receives the declaration?

Not always. The SAAQ's FAQ says the information received is often not enough to decide, so it asks for more details. Except where there is clear danger, it must give 20 days' notice before a suspension takes effect; a person who has had a seizure, for example, may be suspended immediately.

Can a patient sue a professional for reporting them to the SAAQ?

Section 605 of the Highway Safety Code, printed on the form, states that no action in damages may be brought against a health care professional for having used section 603. The SAAQ's FAQ describes this protection as covering reports made in good faith by the designated health professionals.
