SAAQ Eye Examination Report for Drivers (Form 5)

The SAAQ sends this two-page report to a licence holder when their vision must be checked against Quebec's driver health regulation, and an optometrist or ophthalmologist completes it. It records Snellen acuity with and without correction, monocular vision, binocular field anomalies, severe colour vision deficiency, diplopia within the central 40 degrees and listed eye conditions, with a recommendation on night driving.

Who completes it

The examinee signs an authorization at the top; the ophthalmologist or optometrist completes sections 1 to 7 and signs with their licence number.

Submitting and payment

How to submit it
Upload it to the person's SAAQclic account or mail it to the SAAQ's Direction de l'accompagnement et de l'expertise santé.
Who pays
Any fee for completing the form is paid by the examinee and is not reimbursed by the SAAQ.

Good to know

  • If there is a binocular field anomaly, attach field charts signed by the specialist, measured to 160 degrees with Goldmann III/4e with scotoma screening, Esterman or an equivalent technique.
  • Visual field testing must be done with the lenses worn for driving and with both eyes open and examined together.
  • Acuity is reported without telescopic lens correction, in bands from 6/9 or better down to lower than 6/15, and each section cites the matching sections of the regulation.
  • A French-only support guide (6205 25) accompanies the form, and the form points to the SAAQ's explanatory guide to the Regulation respecting the health of drivers.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Can an optometrist complete the SAAQ eye examination report?

Yes. The form covers ophthalmology and optometry, and the examining professional checks either ophthalmologist or optometrist and gives their professional licence number. Diplopia, colour vision, monocular vision and eye conditions such as glaucoma, macular degeneration or diabetic retinopathy are all reported by the same examiner.

What visual field test does the SAAQ accept?

When there is a binocular visual field anomaly, the SAAQ needs binocular field charts signed by the specialist showing measurement up to 160 degrees with the Goldmann III/4e technique with scotoma screening, the Esterman technique or an equivalent recognized technique, done with driving lenses and both eyes open.

When does a Quebec driver need an eye examination report?

The SAAQ sends the form when a driver's vision needs assessment. Its FAQ adds that drivers turning 75 now complete a self-declaration, after which the SAAQ may still request a vision report, while drivers aged 80 and every two years after must provide medical and vision reports.
