SAAQ Neurological Examination Report (Form 14)

The SAAQ sends this two-page report to a licence holder whose neurological health must be assessed for driving. The health care professional describes neurological disorders with cognitive, motor, sensory, balance or coordination effects, then epilepsy, single seizures or non-epileptic seizures, with dates, circumstances and treatment. The report ends with an opinion on whether the SAAQ should require further assessments.

Who completes it

The examinee signs the authorization at the top; the examining health care professional completes sections 1 to 5 and identifies their profession, licence number and how long the patient has been in their care.

Submitting and payment

How to submit it
Submit it through the person's SAAQclic account or by mail to the SAAQ's Direction de l'accompagnement et de l'expertise santé.
Who pays
Any fee for completing the form is paid by the examinee and is not reimbursed by the SAAQ.

Good to know

  • For a single seizure, section 3 asks for an EEG report connected with the seizure, plus its nature, characteristics, circumstances and date.
  • Section 2 asks whether the last seizure followed a physician-directed medication change while epilepsy was controlled, or an exceptional, clearly identified cause unlikely to recur.
  • The examiner can recommend an on-road assessment by an SAAQ examiner or a functional assessment by an occupational therapist, and whether the person should stop driving meanwhile.
  • For toxic, alcohol- or drug-related seizures, section 4 asks since when the person has abstained from the substance that caused them.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Does SAAQ form 14 require an EEG?

For a single seizure, section 3 asks for an EEG report connected with the seizure. For epilepsy, the form instead asks for seizure types, timing (asleep or awake), first and last seizure dates, symptoms, circumstances, medication and dosage, and whether the person adheres to treatment and reports seizures.

Can the SAAQ suspend a licence right after a seizure?

The SAAQ's FAQ says a person who has had a seizure may have their licence suspended immediately. In other situations the SAAQ must usually give 20 days' notice before a suspension takes effect, and it often asks for more information before deciding on a report.

Who pays for the SAAQ neurological examination report?

The examinee. The form states that any fees for completing it are paid by the person examined and are not reimbursed by the SAAQ. The SAAQ sends the form to the licence holder, who has it completed by a health care professional and returned through SAAQclic or by mail.
