SAAQ Psychiatric Examination Report (Form 13)

The SAAQ sends this two-page report to a licence holder whose psychiatric health must be assessed for driving. The health care professional documents the disorder, medical supervision, psychotic episodes over three years and 12 months, treatment and whether the dosage is compatible with safe driving, any DSM substance use disorder, judgment, and which types of vehicle the person can safely operate.

Who completes it

The examinee signs the authorization; the examining health care professional completes sections 1 and 2 and gives their profession, licence number and the period over which they monitored the person.

Submitting and payment

How to submit it
Submit it through the person's SAAQclic account or by mail to the SAAQ's Direction de l'accompagnement et de l'expertise santé.
Who pays
Any fee for completing the form is paid by the examinee and is not reimbursed by the SAAQ.

Good to know

  • Substance use disorder severity is graded by the number of DSM criteria met: mild (2 to 3), moderate (4 to 5) or severe (6 or more), with a remission start date.
  • The examiner answers yes or no for each vehicle type: articulated and non-articulated heavy vehicles, bus, minibus, passenger vehicle, motorcycle, moped and emergency vehicle.
  • The SAAQ does not conduct psychiatric examinations itself; if another professional is to examine the person, the examinee must submit that report by the form's return deadline.
  • Section 2 asks the examiner to describe any situation that may suggest a risk to driving a road vehicle, on a separate sheet if needed.
