Ontario Form 3: Certificate of Involuntary Admission

The attending physician uses this Mental Health Act certificate to admit a patient to an Ontario psychiatric facility as an involuntary patient. The physician certifies a personal examination, that the patient is not suitable for voluntary or informal status, and that the Box A risk-of-serious-harm or Box B criteria are met. The certificate is valid for 14 calendar days, including the day it is signed.

Who completes it

The attending physician completes and signs the certificate after personally examining the patient. The signing physician must then give the patient a Form 30 notice and notify a rights adviser.

Submitting and payment

How to submit it
There is no external submission step on the form; after signing, the physician must promptly give the patient a Form 30 notice and notify a rights adviser, who meets the patient to explain the certificate.

Good to know

  • Box A requires a mental disorder likely to result in serious bodily harm to the patient or another person, or serious physical impairment of the patient, unless they remain in custody.
  • Box B requires all five criteria, including incapacity to consent to treatment with the substitute decision-maker's consent, prior treatment with clinical improvement, and the same or a similar disorder.
  • The rights adviser explains the significance of the certificate and the patient's right to have it reviewed by the Consent and Capacity Board.
  • To continue involuntary status beyond the certificate period, the attending physician completes Form 4, the Certificate of Renewal.
