Ontario Form 45: Community Treatment Order (CTO)

A physician uses this Mental Health Act form to issue or renew a community treatment order (CTO) in Ontario, setting out a community treatment plan for a person who needs continuing treatment or care and supervision while living in the community. The physician confirms the person's psychiatric history, that all criteria are met, that rights advice was given, and that the person or substitute decision-maker consents to the plan.

Who completes it

The examining physician completes Parts 1 and 3 and signs the order. The person, or their substitute decision-maker, completes and signs the Part 2 undertaking to comply with the community treatment plan.

Submitting and payment

How to submit it
Right after signing, the physician gives a copy of the order and plan to the person, their substitute decision-maker, the officer in charge of a psychiatric facility if applicable, and everyone named in the plan, plus a Form 46 notice.

  • The person and any substitute decision-maker must receive rights advice before the order is issued, unless the person refused to consult a rights adviser.
  • The person, or someone on their behalf, can apply to the Consent and Capacity Board on Form 48 to review whether the criteria were met.
