RSP-502A: Retraite Québec Disability Medical Report

Members of Québec public-sector pension plans who are permanently and totally disabled and entitled to a deferred pension can receive disability benefits, and this report supplies the medical evidence. Retraite Québec's medical advisors use it, with the member's education and work history, to decide whether the person can do their usual work or any other suitable work.

Who completes it

The member completes the first part: identification, the pension plan they contributed to, education and work history, and a signed consent. The physician completes the seven-page physician's part, from medical history and diagnosis to ability to work and certification.

Submitting and payment

How to submit it
The member or physician sends both parts to Retraite Québec online through its document-sending service or My Account, or by mail, as soon as possible.
Who pays
The physician's fees are at the member's expense, while Retraite Québec pays for any medical examination it requires.

Good to know

  • Disability follows the Income Tax Act: an impairment that prevents any employment suited to the person's education, training or experience and can reasonably be expected to last for life.
  • For psychiatric conditions, use the DSM multiaxial diagnosis, and rate the prognosis as favourable, stable or unfavourable.
  • Section 7 asks whether other records exist, for example at the CNESST, the SAAQ, an insurer, a hospital, a rehabilitation centre, a physiotherapy clinic or a CLSC.
  • Section 11 lets you identify medical information that should not be shared with the person because it could cause serious harm, and when they could access it.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Is this the same as the Québec Pension Plan disability pension?

No. RSP-502A supports a disability benefit under a public-sector employer pension plan administered by Retraite Québec, such as RREGOP or PPMP, for a member entitled to a deferred pension. It is a pension plan benefit, separate from the Québec Pension Plan disability pension.

What investigations should be attached to the RSP-502A?

Section 6 asks you to specify investigation results and attach copies of relevant supporting documents, such as medical imaging, pathology, exercise ECG or respiratory tests. It also asks whether more consultations or investigations are planned or requested, and when.

Is the RSP-502 available in French?

Yes. The French version, RSP-502, is titled Rapport médical pour une prestation d'invalidité d'un régime de retraite du secteur public. The English RSP-502A was last updated in November 2023 and can be completed on-screen.
