RSP-448A: Retraite Québec Terminal Illness Report

Members of Québec public-sector pension plans administered by Retraite Québec, such as RREGOP, use this report to claim disability benefits in the case of terminal illness. Retraite Québec's health professionals use it to decide whether the member has a terminal illness likely to lead to death within two years of the most recent medical examination, based on the latest medical knowledge.

Who completes it

The member completes the first part (identification and a consent to release medical and psychosocial information) and signs it. The physician completes the physician's part: identification with licence number, medical information, hospitalizations, life expectancy opinion and certification.

Submitting and payment

How to submit it
The member or the physician sends both parts to Retraite Québec online through its document-sending service or My Account, or by mail; Retraite Québec says online sending is faster.
Who pays
The member pays the physician's fees for the report, while Retraite Québec pays if it needs a medical examination of its own.

Good to know

  • Question 5.1 asks directly whether the member's life expectancy is likely to be less than two years, based on the latest medical knowledge.
  • Section 3 asks for the diagnosis and observations on physical and mental condition, first and last examination dates, visit frequency, and full reports and test results.
  • The physician's part can be completed on-screen; Retraite Québec asks that it be returned as soon as possible so the application is processed quickly.
  • The member's consent stays in effect, even after death, until Retraite Québec reaches a final decision, unless revoked in writing.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Is the RSP-448 available in French?

Yes. The French version is the RSP-448, Rapport médical pour une prestation de maladie en phase terminale d'un régime de retraite du secteur public. The English version is numbered RSP-448A, with the current version dated July 2023. Both are on Retraite Québec's website.

What is the life expectancy threshold on the RSP-448A?

Two years. Retraite Québec's health professionals determine whether the applicant suffers from a terminal illness likely to lead to death within the two years following the most recent medical examination. The regular disability medical report, RSP-502A, uses a different test based on permanent and total disability.

What if the member cannot sign the form themselves?

Someone can complete the form on the member's behalf and give their own details. Retraite Québec asks whether it has already received a power of attorney or protection mandate; if not, that document must be enclosed with the application.
