Beneva Prior Authorization Drug Forms (Group Plans)

Beneva calls prior authorization drugs exception drugs: drugs it must authorize before reimbursing them under a group plan. Its online search tool finds the right form for a drug based on the medical condition, alongside a general form (COL-F0344A) and exception forms for generic substitution (COL-F0071A) and the mandatory transition to biosimilars (COL-F0072A). The prescriber's section confirms the drug will be used correctly.

Who completes it

The plan member completes Section A, including any other private or provincial drug coverage, and signs; the prescriber completes Section B for an initial request or Section C for a renewal and signs.

Submitting and payment

How to submit it
The general form lists fax and mail as the ways to submit it, with Beneva's customer service and Client Centre for questions.
Who pays
The general form states the plan member is responsible for any fees charged for completing it.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

How do I find the right Beneva prior authorization form?

Use the search tool on Beneva's exception drugs page: search for the prescribed drug, and Beneva shows which form to fill out based on the medical condition. If no specific form applies, the page offers the general form (COL-F0344A), and a full list of prior authorization drugs is posted.

What is the difference between an initial and a renewal request?

On Beneva's general form, an initial request uses Section B, with the diagnosis, test results before treatment and previous medications with the reason each was stopped. A renewal uses Section C, which compares the initial evaluation with the most recent results to support continuing treatment.

Who pays for completing a Beneva prior authorization form?

The plan member. Beneva's general form states the plan member is responsible for any fees charged for completing it. The member also signs a statement authorizing healthcare professionals and others to share the information Beneva needs to administer the group insurance contract and assess the claim.
