Beneva Critical Illness and Disability Claim Forms

Beneva does not publish blank attending physician statements for individual critical illness, accident-sickness or disability insurance. A claim starts with a short online request, a call to Customer Service or the patient's advisor; Beneva then opens a file, and its claims team contacts the claimant and explains which forms to fill out and return, including any physician statement.

Who completes it

The attending physician completes whatever medical statement Beneva sends once the claim file is opened. The insured person, or someone acting for them, fills out the online request with identification, the claim type, the policy number and a brief description of the need.

Submitting and payment

How to submit it
The online request is the first step for individual policies, while coverage through an employer, union or association goes through Beneva's group insurance claims page. Beneva warns that sending personal information by email carries risks.

Good to know

  • The online request covers three claim types: critical illness, accident-sickness, and disability or disability benefits.
  • Beneva uses the insured person's date of birth and policy number to find the file; policy numbers have 9 or 10 digits or start with S, A or 007.
  • Beneva says it needs information to identify the file before sending any form to fill out, so physician statements come after the request.
  • Group coverage through an employer, union or association follows Beneva's separate group insurance claims process rather than this individual request.

Making a critical illness, accident-sickness or disability claim

Fill it with Vero

If your copy of the form is a fillable PDF, upload it once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Where can I download Beneva's attending physician statement for a critical illness claim?

Beneva does not post a blank physician statement for individual critical illness, accident-sickness or disability claims. The insured person first submits Beneva's online request, or calls Customer Service or their advisor; Beneva opens a file and its claims team explains which forms must be filled out and returned.

What information does Beneva's online claim request ask for?

It asks who the request is for, the insured person's name and date of birth, contact details, the relationship of anyone filing for someone else, the claim type (critical illness, accident-sickness, or disability), the policy number and a brief description of the need. Beneva then gets in touch as soon as possible.

Does this process apply to group insurance through an employer?

No. Beneva's life and health claims page directs people whose coverage is through an employer, union or association to its group insurance claims page. The online request and advisor route described here apply to individual critical illness, accident-sickness and disability policies.
