RAMQ Form 4086: Medical Exemption, Photo or Signature

Physicians complete this French certificate when an illness or impairment makes it impossible for an insured person to have their photo taken, to sign, or both, for the Québec health insurance card, which normally carries both for people aged 14 to 74. The patient attaches the certificate to their card form so the card can be issued without the photo, the signature or both.

Who completes it

The physician completes Sections 2 to 5: the nature of the illness or impairment, whether the incapacity is permanent or temporary (with an approximate duration), the exemption requested, and their name, professional number, signature, date and the establishment's stamp if any. Section 1 identifies the insured person.

Submitting and payment

How to submit it
The patient attaches the certificate to their health insurance card form; with a photo and signature exemption everything can be mailed to RAMQ, but a photo-only or signature-only exemption still requires identification at a service point.
Who pays
RAMQ states that the patient pays any fees for obtaining this medical certificate.

Good to know

  • A combined photo and signature exemption means the person does not have to appear at a service point to be identified, and RAMQ returns original documents sent by mail.
  • Children under 14 are exempt and photo and signature are optional from age 75, so the certificate mainly concerns insured people aged 14 to 74.
  • Separately, RAMQ's photo rules require a medical certificate for people with photophobia or light intolerance who need to wear dark or tinted glasses in their card photo.
  • The current version is 4086 365 17/12, updated in RAMQ's forms index on December 14, 2023, and is a French-only printable PDF.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can sign RAMQ form 4086?

A physician. RAMQ's photo and signature page tells the person to have a physician complete the Certificat médical pour l'exemption de photo ou de signature, giving the physician's name, professional number, signature and date, along with the nature of the illness and the duration of the incapacity.

Is a health insurance card photo exemption permanent?

It depends on the physician's answer in Section 3, which classes the incapacity as permanent or temporary, with an approximate duration for temporary cases. Section 4 then sets the scope of the exemption: photo and signature, signature only, or photo only.
