RAMQ 3486: Medical Certificate, Hearing Aids Program

Under RAMQ's hearing aids program, an otolaryngologist uses this French certificate to document the type of hearing loss in each ear, whether the impairment is permanent, and any medical contraindication to a hearing aid. The specialist also states, ear by ear, whether wearing a hearing aid is necessary. The certificate supports coverage of a hearing aid or an assistive listening device.

Who completes it

The otolaryngologist completes the whole certificate and signs it with their professional number and the date; the only patient details are name and health insurance number. An audiologist completes separate documents, such as the 3485 recommendation for assistive listening devices.

Submitting and payment

How to submit it
The form gives no mailing instructions; RAMQ's program page says the patient brings the required documents to a Québec hearing aid acoustician (audioprothésiste) or assistive device distributor within one year of their issue.
Who pays
The program covers hearing aid acousticians' and distributors' fees for supplying, replacing and repairing covered aids; the certificate lists no fee for the otolaryngologist.

Good to know

  • For people aged 12 to 64, RAMQ asks for an audiogram and an attestation that a hearing aid is needed, both obtainable from an otolaryngologist or an audiologist.
  • Children 11 and under need an audiologist's attestation, and people 65 and over need an audiologist's audiogram, according to RAMQ's hearing aids page.
  • A second covered hearing aid needs an audiologist's recommendation and is limited to people 18 or under, people with a visual impairment, or adults who need it for school or work.
  • Covered hearing aids are in-the-ear and behind-the-ear models; replacement is covered for changes in hearing or physical condition, accidental breakage, or repairs above set cost thresholds.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who completes RAMQ's medical certificate for hearing aids?

An otolaryngologist (oto-rhino-laryngologiste). The certificate records the type of deafness in the right and left ears, whether the impairment is permanent, any medical contraindication to a hearing aid or assistive listening device, and whether a hearing aid is necessary for each ear, followed by the specialist's signature and professional number.

What hearing loss qualifies for RAMQ's hearing aids program?

Adults 19 and over need an average loss of at least 35 decibels in the better ear at the required frequencies, while people 12 to 18, and adults who are studying, need at least 25 decibels in one ear. Children 11 and under qualify with a loss likely to compromise speech and language development.

Is form 3486 available in English?

No. RAMQ publishes the Certificat médical, Programme d'aides auditives in French only, version 3486 285 15/01. The patient's name and health insurance number appear at the top, and the rest is completed and signed by the otolaryngologist.
