RAMQ Form 3841: Need for a Mobility or Posture Aid

Rehabilitation establishments use this six-page French form to justify a manual or power wheelchair, positioning base, adapted stroller or posture aid under RAMQ's program for devices that compensate for a physical deficiency. An occupational therapist or physiotherapist documents a full functional evaluation and intervention plan. The authorized prescribing physician co-signs, and the establishment's technical aids service supplies the device.

Who completes it

The occupational therapist or physiotherapist completes the request (aids requested, nature of the request, eligibility, justification) and the functional evaluation, attesting that the person is eligible and needs the aid. The authorized prescribing physician signs Section 8 with their name and professional number.

Submitting and payment

How to submit it
The form is completed within an authorized public rehabilitation establishment's technical aids service, which supplies the device; it records the dispensing establishment and, if different, where the evaluation took place.
Who pays
RAMQ's program covers the purchase, replacement and repair of insured mobility and posture aids supplied by authorized establishments; no professional fee appears on the form.

Good to know

  • Section 4 needs a medical diagnosis confirming the physical deficiency is permanent, the organ-system deficiency that justifies the device's complexity, and whether there is a degenerative musculoskeletal or neurological disease.
  • For a power wheelchair, the form asks the clinician to justify the deficiency that prevents manual propulsion, not the one that prevents walking.
  • Special consideration (C.S.) aids or components need extra justification showing, through trials, that no device or component listed in RAMQ's tariff meets the person's mobility or posture needs.
  • The functional evaluation covers self-care, transfers, sitting tolerance, pressure relief, Berg balance, walking and propulsion capacity, transport, home, school or work environment, and seated posture.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who signs RAMQ form 3841?

Two people: the occupational therapist or physiotherapist who did the functional evaluation, attesting that the person is eligible for the program and needs the requested aid, and the authorized prescribing physician, who signs Section 8 with their name in block letters and professional number.

Which devices does form 3841 cover?

Manual wheelchairs, power wheelchairs, positioning bases, adapted strollers and posture aids, including special consideration (C.S.) devices or components. The request can be an initial supply, an additional device or a replacement; for a replacement, the form records the date of possession and model of the device being replaced, and the reason.

What if the person lives in a care facility?

Section 4 asks whether the person lives in a public or subsidized private (conventionné) establishment and, if so, whether they have special needs that a standard manual wheelchair cannot meet. The answer forms part of the eligibility section completed by the therapist.
