RAMQ 3485: Assistive Listening Device Recommendation

Under RAMQ's hearing aids program, an audiologist uses this French-only form to recommend assistive listening devices for an eligible insured person after a global assessment of hearing needs. It covers three device groups: text transmission, sound transmission and environmental alerting detectors. The patient then takes the recommendation to a Québec distributor of assistive listening devices within one year of the documents being issued.

Who completes it

The audiologist completes the whole form: patient identification, the date of the global hearing needs assessment, pure-tone averages at 500, 1000, 2000 and 4000 Hz for each ear, the recommended device types with remarks, and their signature and professional number. RAMQ also requires a global assessment of impairments and functional limitations from an audiologist or speech-language pathologist.

Submitting and payment

How to submit it
The form prints in three copies, for RAMQ, the distributor and the audiologist; the patient brings the recommendation and other required documents to a Québec assistive device distributor within one year of their issue.
Who pays
The program pays distributors' fees for supplying, replacing and repairing covered aids, so the patient pays nothing for those services; the form lists no fee for the audiologist's recommendation.

Good to know

  • For sound transmission aids, the audiologist ticks whether the person has no hearing aid or cannot use one, or has already used at least one hearing aid for over a month.
  • The form's note asks for proof of school attendance and/or an employment attestation with a job description when required, for example for students and workers.
  • Covered assistive listening aids include telephone amplifiers, door or phone ring detectors and adapted alarm clocks; lost, stolen or neglected aids are not covered by the program.
  • Eligibility depends on age and loss: adults 19 and over need an average loss of at least 35 dB in the better ear, or at least 25 dB in one ear if studying.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can recommend an assistive listening device under RAMQ's program?

An audiologist signs form 3485. RAMQ's hearing aids page lists two documents for an assistive listening device: a global assessment of impairments and functional limitations from an audiologist or speech-language pathologist, and the recommendation of the aid from an audiologist.

Is form 3485 available in English?

No. RAMQ publishes it in French only (Recommandation, aide de suppléance à l'audition; version 3485 285 15/01). It prints in three copies, one each for RAMQ, the distributor and the audiologist, and sits alongside the otolaryngologist's medical certificate (form 3486) in the same program.

How long is the recommendation valid?

RAMQ's hearing aids page says the patient must meet the distributor, bringing the required documents and any proof of studies, work or another impairment, no later than one year after the required documents are issued.
