Revenu Québec TPZ-1029.MD.A: Dependent Senior Status

Seniors claiming Québec's tax credit for home-support services who must prove they are dependent use this one-page certification. A physician or specialized nurse practitioner answers two questions in Part 2, on permanent dependence for personal care and on constant supervision for a serious mental disorder, and gives the date the patient became dependent. A Yes to either question certifies dependent senior status.

Who completes it

The physician or specialized nurse practitioner completes Part 2, Medical evaluation, and signs the Part 3 certification with their provincial licence number. The senior completes Part 1 with their identification and address.

Submitting and payment

How to submit it
The completed form is mailed to Revenu Québec at the Québec City address printed at the top of the form.

  • If you answer Yes to either question, enter the date on which, to the best of your knowledge, the patient became dependent.
  • The form is fillable, published in English (TPZ-1029.MD.A-V) and French (TPZ-1029.MD.A), and Revenu Québec accepts an electronic signature on it.
