Revenu Québec TP-1029.8.66.2M: Infertility Certificate

Québec taxpayers claiming the refundable tax credit for the treatment of infertility, or applying for an advance payment of it, use this one-page certificate. The physician carrying out the treatment confirms whether equivalent treatment is offered at a centre for assisted procreation within 200 kilometres and whether the person can travel without assistance, and names the centre and its licence number.

Who completes it

The physician carrying out the treatment completes Part 3, the certification, and signs Part 4. The person claiming the credit completes Part 1 and, in Part 2, identifies who is undergoing treatment: themselves, their spouse or another person.

Submitting and payment

How to submit it
The certificate is provided with the infertility tax credit claim (TP-1029.8.66.2) or with the advance payment application (TP-1029.8.66.2A).

  • Part 3 asks for the name, address and licence number of the centre for assisted procreation where the treatment is being carried out.

The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.
