TP-1029.AN.A: Certificate of Ongoing Assistance

Caregivers claiming Québec's tax credit for caregivers for someone they are not related to need this three-page certificate, one for each care receiver. The caregiver and the care receiver each consent and certify the arrangement, and a health professional confirms in Part 5 that the caregiver provides ongoing assistance so the care receiver can carry out a basic activity of daily living.

Who completes it

A professional in the health and social services network, or a private-sector professional who is a member of a professional order and mandated by the network to serve the care receiver, completes and signs Part 5. The caregiver completes Parts 1 and 2, and the care receiver Parts 3 and 4.

Submitting and payment

How to submit it
The caregiver encloses the certificate with the tax credit claim, or sends it to Revenu Québec with a signed letter giving their social insurance number and stating that they claimed the caregiver credit.

Good to know

  • A Certificate Respecting an Impairment (TP-752.0.14-V) must already confirm that the care receiver has a severe and prolonged impairment and needs help with a basic activity of daily living.
  • The certificate must be renewed every three years, and a separate certificate is needed for each eligible care receiver the caregiver helps.
  • Revenu Québec may contact the health and social services network to confirm the assistance; if the caregiver and care receiver do not consent, the return will not be processed.
  • Part 5 asks for your professional category and provincial licence or professional order member number, and Revenu Québec accepts an electronic signature on the form.
