TP-752.0.13.1: Medical Services Not in Your Area

Taxpayers file this four-page form with their Québec income tax return to claim, on line 378, travel, lodging or moving costs paid to obtain medical services in Québec that were not available within 200 kilometres of home. The attending physician or a specialized nurse practitioner certifies the services provided and that equivalent services were not available closer.

Who completes it

For travel or lodging, the attending physician of the person who received the services, or a specialized nurse practitioner, completes Parts 3 and 4; for moving expenses, they complete Part 6 and certify Part 7 along with a health establishment's chief administrator. The taxpayer completes the remaining parts.

Submitting and payment

How to submit it
The taxpayer files the completed form with their Québec income tax return; receipts are not enclosed but must be kept in case Revenu Québec asks for them.

Good to know

  • Part 3 asks whether equivalent or almost equivalent services were available in Québec within 200 kilometres on the same dates, and whether a patient aged 18 or older could travel alone.
  • An accompanying person's travel and lodging count only if the patient was under 18 during the year or unable to travel alone.
  • For moving expenses, you also certify that the medical services can reasonably be expected to last at least six months, and the new home must be within 80 kilometres of the establishment.
  • Travel or lodging for purely cosmetic medical or dental services does not qualify, nor do amounts already claimed as medical expenses; Revenu Québec accepts an electronic signature on the form.
