CNESST 5053: Rapport de physiothérapie

CNESST asks the treating physiotherapist for this four-page, French-only report on a worker receiving physiotherapy for an employment injury on the treating professional's prescription. It compares the worker's initial and current condition, covering pain ratings, objective findings, functional tolerances and return-to-work obstacles, then sets out the treatment plan and whether treatment should end. The report also serves as the invoice.

Who completes it

A member of the Ordre professionnel de la physiothérapie du Québec completes and signs the whole report, with their member number and the clinic's CNESST supplier number. The treating health professional's prescription date and diagnosis are recorded on it, and that professional receives a copy.

Submitting and payment

How to submit it
CNESST must receive it within 15 days of the request date shown in section 1, sent through certified clinic software, the Guichet SST, the online document-sending service, fax or mail, with a copy to the treating health professional.
Who pays
CNESST pays a set fee per report under the 'Rapports' line of its physiotherapy and occupational therapy tariff when its own form or a certified-software version is used, and no amount may be claimed from the worker.

Good to know

  • If CNESST hasn't received the report 15 days after its request, it suspends payment for treatments until the report arrives; the suspension doesn't apply to public health establishments.
  • When only a physiotherapy technologist treats the worker, a physiotherapist or the treating professional must send a report after the first 25 treatments and every additional 12.
  • Once the worker reaches a functional recovery plateau, the physiotherapist must suspend treatment and refer the worker back to the treating health professional.
  • IVAC uses the same form for crime victims receiving physiotherapy, with the report due 15 days after IVAC's request.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

What happens if the CNESST physiotherapy report is late?

The report must reach CNESST no later than 15 days after the request date printed in section 1. After that, CNESST suspends payment for the worker's treatments until it receives the report. The suspension does not apply to public health establishments. Clinics can send it through certified software, the Guichet SST, the online document-sending service, fax or mail.

What pain scale does the 5053 use?

Section 3 records the worker's pain intensity at rest, in movement and on palpation, each out of 10, where 1 is the lowest level and 10 the highest. The same section asks which positions or movements are affected, whether daily and work activities are affected, and how the worker sees their return to the pre-injury job.

Can the worker be charged for the physiotherapy report?

No. The form's notes cite section 194 of the Act respecting industrial accidents and occupational diseases: the provider cannot claim any amount from the worker. CNESST pays the report under its tariff, and the report itself serves as the invoice, provided it is on CNESST's form or one produced by CNESST-certified software.
