CNESST 5008: Compte de soins, psychologie

Psychologists, psychotherapists and neuropsychologists bill CNESST on this three-page form for care given to an injured worker. Beyond the invoice, its Informations communes aux rapports page must be attached to every evaluation, progress and final report sent to CNESST. Fees follow the hourly rate in Quebec's regulation on health services, with caps on the time paid for writing each report.

Who completes it

The psychologist, psychotherapist or neuropsychologist completes the whole form, including the GST and QST registration numbers. Both the clinic and the professional must be authorized CNESST suppliers.

Submitting and payment

How to submit it
Send it through CNESST's Envoi de document service or by fax, with the original prescription.
Who pays
CNESST pays the hourly rate set by the Règlement sur les services de santé, l'équipement adapté et les autres frais, or the contract rate for services under a CNESST contract.

Good to know

  • Report-writing time is capped: 2 hours for an evaluation report (8 hours in neuropsychology), 1 hour for a progress report and 2 hours for a final report.
  • Attach the Informations communes aux rapports page to each report: the evaluation report (5005-R), progress report (5006-R) and final report (5007-R).
  • Services provided under a contract with CNESST are paid at the contract rate rather than the regulation's hourly rate.
  • The current version is dated 2025-12: a three-page fillable PDF with 91 fields, published in French only.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

How much report-writing time does CNESST pay psychologists?

Up to 2 hours for an evaluation report, or 8 hours for a neuropsychological evaluation, 1 hour for a progress report and 2 hours for a final report, at the hourly rate set by the Règlement sur les services de santé, l'équipement adapté et les autres frais. Services under a CNESST contract are paid at the contract rate instead.

What is the Informations communes aux rapports page?

It is a page of CNESST form 5008 that must be attached to each evaluation, progress and final report a psychologist, psychotherapist or neuropsychologist sends to CNESST. The reports themselves are separate forms, 5005-R, 5006-R and 5007-R, while form 5008 also serves as the invoice for the care given.

Do I need to be registered with CNESST to bill psychology services?

Yes. Both the clinic and the professional must be authorized CNESST suppliers, and the form asks for GST and QST registration numbers. Send the completed form through the Envoi de document service or by fax, with the original prescription for the psychological care.
