Hawaii WIC Form FD 141C Medical Documentation

Hawaii WIC needs this form before it issues a non-contract formula to a participant with a qualifying medical condition or tailors their WIC foods. The provider completes sections A to D for every patient: diagnosis and how long the documentation is valid, formula and daily amount, supplemental foods and milk type, and signature. Page 2 lists approved formulas and their indications.

Who completes it

An MD, DO, NP or PA completes sections A to D and signs. WIC staff fill in the clinic contact details at the top and record who approved the request.

Submitting and payment

How to submit it
Fax the completed form to the patient's WIC clinic or give it to the family to bring to WIC. The top of the form has space for the WIC clinic name and contact person.

Good to know

  • Colic, constipation, spitting up and formula intolerance are not qualifying conditions. The provider chooses how long the documentation is valid, from 1 to 6 months.
  • Contract formulas are Similac Advance, Sensitive, Soy Isomil and Total Comfort or Gentle Comfort. Enfamil Gentlease is given, with this form, only when Total Comfort is unavailable.
  • If no food box is checked, the WIC dietitian or nutritionist decides. Whole milk goes to children over 2 and women only with a prescribed formula.
  • Hawaii WIC cannot provide supplementation formulas, Good Start standard formulas, Enfamil NeuroPro Infant or ProSobee. Metabolic formulas are supplied, but WIC doesn't provide medical nutrition therapy.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
