Arkansas WIC-51 Medical Documentation Form

Arkansas WIC uses this one-page form to authorize special formulas, medical foods and food package changes that federal rules allow only with medical documentation. The prescriber picks the formula from a checklist, states the qualifying condition, amount and length of issuance, and notes any food restrictions. A WIC registered dietitian approves the request under program policy.

Who completes it

An MD, DO, APRN or PA completes sections A to D (items 1 to 15) and signs. WIC staff fill in the household ID, CPA name, approving dietitian, and start and expiration dates.

Submitting and payment

How to submit it
Fax the form to the patient's local WIC clinic or give it to the family to bring to their WIC appointment. WIC staff may contact the office to request more information.

Good to know

  • Length of issuance options are 3, 6 or 12 months or another period, with a box to discontinue a prescribed formula. Amounts can be the maximum allowed or 24, 16 or 8 ounces a day.
  • The diagnosis checklist covers prematurity with weeks of gestation, cow's milk protein allergy, GERD, short bowel syndrome, cystic fibrosis, PKU, failure to thrive and tube feeding, plus an 'Other' line.
  • Ready-to-use or concentrate needs a reason: unsafe or limited water supply, improper formula preparation, prematurity or tube feedings.
  • Section C keeps the standard food package or adjusts it, for example omitting infant foods until a date, giving soy milk to a child under 2 or whole milk after 2.
