Indiana WIC Medical Documentation Form

Indiana WIC needs this documentation before it issues exempt infant formula, WIC-eligible nutritionals or tailored foods for a patient with a qualifying condition. The provider checks the conditions, names the formula, daily amount and physical form, chooses the allowed foods by age, and sets a length of use. WIC staff scan the form into the client's record.

Who completes it

The health care provider completes each section and signs, adding the medical office, phone and address. Before WIC faxes anything back to the provider, the client signs an Indiana WIC Release of Information at the clinic.

Submitting and payment

How to submit it
Give the completed form to the family to provide to their WIC local agency, or fax it to the clinic, where staff scan it into the client's record.

Good to know

  • Formula or food intolerance, preference, milk or soy protein allergy not needing an elemental formula, and weight management without a medical condition are not qualifying conditions.
  • Length of use is 1, 3, 6 or 12 months, with 12 the maximum approval. A length is required even when only food alternatives are ordered.
  • Checking yes on comparable formula lets WIC issue another product of the same type, listed on page 2, during a recall or supply problem.
  • Children 12 to 24 months get whole milk, and older children and women 1% or skim, unless the form orders a different milk for a qualifying condition.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How long can an Indiana WIC medical documentation order last?

The provider chooses 1, 3, 6 or 12 months, with 12 months the maximum approval, or writes another length. A length of use is required even when the order covers only food alternatives, such as pureed fruits and vegetables or a different milk, with no formula.

Can Indiana WIC give a different formula if the prescribed one is unavailable?

Only if you authorize it. Section 2 asks whether WIC may issue a comparable formula when the prescribed one is hard to find, for example during a recall or supply chain problem. If yes, staff may substitute within the same formula type, such as another extensively hydrolyzed or amino acid based infant formula.
