Washington F252-056-000: Cervical Impairment Worksheet

Washington doctors rating permanent impairment of the cervical or cervico-dorsal spine use this two-page worksheet. After confirming maximum medical improvement, aggravation of any pre-existing condition and permanent impairment, the doctor circles one box in each of three columns (nerve root involvement, neck rigidity substantiated by imaging, and range of motion or spasm findings), then averages them into the category rating.

Who completes it

The attending physician or consultant performing the rating completes and signs it, certifying an exam within the last 8 weeks. Under L&I policy, chiropractors must be approved IME examiners to rate impairment.

Submitting and payment

How to submit it
Send it to L&I with the closing or impairment rating report and supporting records, and keep a photocopy for your records.
Who pays
Impairment ratings are billed under L&I's Chapter 11 codes, for example 1191M for an attending physician's standard rating of 1-3 body areas, with office visits bundled into the rating fee.

Good to know

  • Page two gives a worked example of a patient with neck pain, a positive foramina compression test and disc bulges, showing how each column is scored.
  • Age-related imaging changes may not be significant in some cases; include only findings consistent with the clinical picture.
  • If the lower extremities are involved, such as paraparesis, consult the Medical Examiners' Handbook; bladder and bowel sphincter impairment is rated separately.
  • Lumbar and lumbosacral impairment uses a separate worksheet, F252-006-000, which has four findings columns instead of three.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How is the cervical category rating calculated?

Circle one box in each of columns A, B and C, add the numbers, divide by 3 and round to the nearest whole number (1.1 becomes 1, 1.5 becomes 2). You can instead copy the circled numbers into the boxes and let L&I do the calculation.

Do pain scales count on the cervical worksheet?

No. Oswestry and other pain scales are not considered. Pain is part of the rating only when it shows up in findings recorded on the worksheet, such as decreased range of motion, which belongs in column C with spasm and other findings.
