WA L&I F242-387-000: IME Physical Capacities

When Washington L&I or a self-insurer asks an independent medical examiner for physical capacities, the examiner records them on this one-page estimate. Based on the clinical evaluation and other test results, it shows how long the worker can sit, stand and walk in an 8-hour day, lifting and carrying limits by weight and frequency, repetitive hand and foot use, postural activities and environmental restrictions.

Who completes it

The L&I-approved independent medical examiner completes and signs the form, marking N/A for any item they cannot answer. When several examiners perform one exam, only one of them bills for the estimate.

Submitting and payment

How to submit it
Return it with the IME report to L&I or the self-insurer that requested the exam.
Who pays
It is billed with local code 1105M, which must be requested by the insurer and is billed once per exam, even when multiple examiners took part.

Good to know

  • Frequency is a percentage of the workday: seldom 1-10%, occasionally 11-33%, frequently 34-66% and continuously 67-100%, with lifting and carrying weights from up to 5 lbs to 51-100 lbs.
  • Restrictions on lifting from the floor or overhead belong in the Remarks section, along with other functional limitations the grid does not cover.
  • The last question asks whether the worker may be tested to tolerance if a performance-based physical capacities evaluation is requested, and if not, what restrictions apply.
  • Only department-approved IME providers with an IME provider account number can bill IME codes, and prior authorization is required when an IME is scheduled.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When is Washington's IME physical capacities form completed?

When the insurer requests it as part of an independent medical exam. L&I's fee schedule says the estimate, local code 1105M, must be requested by the insurer, so examiners complete it when the assignment asks for physical capacities and return it with the IME report.

What if I can't assess an item on the IME physical capacities form?

Mark it N/A. The form's instructions say to complete each item based on your clinical evaluation and other testing results, and to mark N/A for any item you do not believe you can answer. Percentages refer to an 8-hour workday.
