Washington F245-058-000: L&I IME Report Template

Washington L&I publishes this Word template as a standard structure for independent medical examination reports. It takes the examiner from identifying information and history through record review, physical examination, diagnostic studies and pain inventories to conclusions on diagnoses, work-relatedness, prognosis, restrictions and recommendations, an impairment rating report and answers to the claim manager's specific questions.

Who completes it

The L&I-approved independent medical examiner writes the report on the template; it has signature blocks for two examiners, each with name, specialty and date, for exams with more than one examiner.

Submitting and payment

How to submit it
Send the finished report to L&I or the self-insured employer that scheduled the IME.
Who pays
IME fees follow L&I's Chapter 11 payment policy, for example local code 1108M for a standard exam of 1-3 body areas and 1109M for 4 or more, with separate codes for addenda and extensive file review.

Good to know

  • List the accepted conditions exactly as stated in the claim manager's assignment letter, then separate pre-existing conditions from those acquired after the injury or exposure.
  • The impairment rating section mirrors L&I's required elements: maximum medical improvement, physical exam, diagnostic tests, the rating and its rationale.
  • Job analysis reviews belong in the report; standard and complex IME fees include up to two job analyses reviewed on the exam date.
  • When the insurer also asks for physical capacities, examiners complete the separate estimate form F242-387-000 alongside the report.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Do I have to use L&I's IME template?

L&I offers it as the standard structure for IME reports, and its payment policy requires IME reports to contain the elements in the Medical Examiners' Handbook. Following the template's headings, from history and record review to impairment rating and the claim manager's questions, helps make sure none is missed.

What goes in the conclusions of an L&I IME report?

The template lists accepted conditions from the assignment letter, diagnoses with pre-existing and post-injury conditions separated, discussion of work-relatedness, prognosis, physical restrictions, review of job analyses, diagnostic, therapeutic and follow-up recommendations, and findings unrelated to the accepted condition.
