Washington F242-397-000: L&I Preauthorization Request

Washington providers send this request when a State Fund workers' compensation service needs claim manager preauthorization. For up to two diagnosis and procedure pairs, it lists codes, whether the diagnosis is currently allowed, side of body or tooth number, purpose, estimated dates and a short causal-relationship explanation. Objective medical documentation, such as chart notes or test results, is attached to support coverage.

Who completes it

The requesting provider, whether attending, consulting or other, or the office staff complete the whole form, including the L&I provider ID or NPI and a staff contact at the office.

Submitting and payment

How to submit it
Fax it to the L&I number printed on the form or the fax number you normally use for L&I medical correspondence. The form is for State Fund claims only.
Who pays
The request itself carries no fee; it is how the claim manager approves coverage of a service before it is delivered.

Good to know

  • Do not use it for services that need utilization review through Comagine, such as inpatient surgery or MRIs; L&I's Authorization & Referral Fee Schedule tool shows which route applies.
  • Fields have character limits, 125 for names and descriptions and 175 for the causal relationship, so keep entries short and put detail in the attachments.
  • Therapy visit extensions go on the Provider Hotline form (F245-417-000), and providers outside the United States use F245-429-000.
  • A Spanish version of the request (F242-397-999) is also available from L&I.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
