Washington F242-208-000: LEP Application (Medical)

Washington injured workers apply for loss of earning power (LEP) benefits, a partial wage replacement, with this one-page packet. The worker reports hours and gross earnings, the employer certifies wages paid, and the attending physician confirms the work-related diagnosis and whether the disability limits the worker to modified duty or reduced hours. The worker then mails the finished packet to L&I's Claims Section.

Who completes it

The attending physician completes the physician's section: diagnosis, modified duty or reduced hours, restrictions, return to the pre-injury schedule, factors impeding recovery, MMI and permanent impairment. The worker completes and signs the worker's section, and the employer completes its section or attaches payroll records.

Submitting and payment

How to submit it
The worker gathers the completed sections and mails the packet to L&I's Claims Section at the address on the form. Questions go to the claim manager.
Who pays
L&I's payment policy lists the Loss of Earning Power form under local code 1027M, which must be requested by the insurer and is payable only to the attending provider on the claim, once per day.

Good to know

  • When a vocational counselor is involved instead, L&I uses a separate VOC version (F242-209-000) in which the counselor reports retraining, modified duty, reduced hours or reduced wages.
  • The physician states whether the worker has been advised to return to the pre-injury schedule or duties and, if not, when that is expected.
  • Factors impeding recovery, such as unrelated medical conditions, socio-economic issues or chemical dependency, are marked yes or no and explained on extra sheets if needed.
  • L&I offers a Spanish edition (F242-208-999), a bilingual edition and more than 50 other translations of the application.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who completes the physician section of Washington's LEP application?

The attending physician. L&I's payment policy says only the attending provider on the claim may sign and be paid for the LEP form, and only when the insurer requested it. The worker and employer complete their own sections before the worker mails the packet to L&I.

What does the LEP physician section ask?

The diagnosis due to the workplace injury or illness; whether the disability allows only modified or lighter duty or reduced hours, with hours and days per week; the physical restrictions; return to pre-injury work; impeding factors; whether the condition has reached maximum medical improvement; and whether permanent impairment will result.
