TRICARE East Banked Donor Milk Request Form

TRICARE covers banked donor human milk for infants with certain serious health conditions, and Humana Military uses this one-page courtesy form to collect what East Region initial and reauthorization requests need. The ordering provider confirms that the mother's milk is contraindicated, unavailable or insufficient, identifies the infant's qualifying condition, and attaches the form to the referral in provider self-service.

Who completes it

The ordering provider, typically the infant's neonatologist or pediatrician, completes the form with the beneficiary's name, date of birth, DoD benefit number and the provider's tax ID or NPI, and supplies the 30-day prescription.

Submitting and payment

How to submit it
Attach it to the TRICARE referral submitted through Humana Military's provider self-service; fax is used only when online submission is not possible.
Who pays
Coverage is limited to 35 ounces per day per infant, and the milk must come from an HMBANA-accredited milk bank through a TRICARE-authorized provider.

Good to know

  • The initial prescription must state the quantity and frequency of donor milk and is valid for only 30 days; later prescriptions must be renewed every 30 days.
  • Qualifying conditions include birthweight under 1,500 grams, digestive disorders or surgery, failure to thrive after other feeding options, formula intolerance, low blood sugar, heart disease and organ transplant.
  • Milk must come from a Human Milk Banking Association of North America accredited bank, delivered through a TRICARE-authorized pediatrician, inpatient hospital or HMBANA-accredited supplier.
  • The form is optional, but Humana warns that missing clinical information can delay or terminate authorized care or lead to denials of pended claims. Current version XPAF0525-A.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

Which infants qualify for TRICARE banked donor milk?

Infants whose mother's milk is contraindicated, unavailable or insufficient, and who have a condition such as birthweight under 1,500 grams, a digestive disorder or surgery, failure to thrive, formula intolerance with feeding difficulty or weight loss, low blood sugar, heart disease, organ transplant, or another serious condition where donor milk is medically necessary.

How long is a TRICARE donor milk prescription valid?

The initial prescription, stating quantity and frequency, is valid for 30 days. Each subsequent prescription must also state quantity and frequency and be renewed every 30 days, which is why the form covers both initial and reauthorization requests.

Is the Humana donor milk form required?

No. Humana Military provides it as a courtesy, and providers are not required to use it. However, the referral must include the clinical information it lists, because missing information can delay or terminate authorized care or cause pended claims to be denied.
